Contact details and current address.
purpose of the trip (tourism, business or family visit).
the arrival flight number (this can be found on your boarding pass).
Name and address of the hotel (or home address if staying with friends or family).
Days to spend there.
Details on any items you could be bringing into the country.
This form will be compared to the printed documents provided in the section above, so fill it out completely.
