Following is a list of what you'll need to apply for a Czech Republic visa for a short stay:
Visa request type for the Czech Republic. Fill out the form digitally, print it off, and sign it at the end.
I've attached two images for your passport that are duplicates. For the Czech Republic to meet the photograph criteria for a Schengen visa, the photos must have been shot within the recent three months.
Acceptable passport. Ensure that your passport has at least two blank pages where the visa sticker can be affixed before applying. It should be no more than 10 years old and valid for at least three months after the day you want to leave the territory of Schengen, including the Czech Republic.
A copy of your previous visas. Send copies of any visas you've ever had for countries worldwide.
The Schedule of a Return Flight is a confirmed reservation that includes departure and arrival times from and to the Czech Republic and the appropriate flight numbers.
A comprehensive travel medical insurance policy. A certificate attests that you have medical coverage when visiting the Czech Republic and the rest of the Schengen Area. A minimum of 30,000 Euros in coverage for medical emergencies is required here.
Documentation of reasonable accommodations. Documentation of where you plan to stay in the Czech Republic. That is, booking a room in a hotel in the Czech Republic for the full length of your planned visit. Look for and Reserve Accommodations in the Czech Republic Now!
Evidence of having enough money to cover expenses. The Czech Republic requires proof of funds equal to about 42 Euros for each day you wish to stay on its territory.
The cover letter. A letter explaining your interest in travelling to the Czech Republic, your intended length of stay, and your intended activities during your time there.
A valid proof of citizenship. A marriage licence, birth certificate, death certificate, and so on all qualify.
